New Patients
We are pleased whenever a new patient joins our practice. In an effort to stay on schedule, we request that new patients arrive five minutes early for the first appointment in order to complete the necessary forms. We will need the insurance card and if necessary, we will need a referral.

If an appointment needs to be cancelled or rescheduled,
we would appreciate at least 24-hours notice.
